    It could be an allergy, or a severe asthma attack brought on by an allergen, even a severe case of epiglottitis. Most epi shots are related to anaphylaxis or an allergy of some type. There are other indication for epinephrine, such as a myocardial infarction, especially one related to congestive heart failure but these are not usually given in the hypogastric region. Side effects of an epinephrine shot includes tachycardia, sweating, feeling light-headed or feeling the "adrenaline rush".
